European health systems faces a major dilemma: maintaining social protection systems established after the Second World War, based on solidarity between generations and social classes, or facilitate the conversion of health in a commodity subject the laws of the market. In the latter, the “Management science” means not only an instrument to improve the productivity of organizations, but mainly the application of an ideology that permeates the organizations. Paradoxically, the available empirical evidence, after more than twenty years of application worldwide, calls into question its ability to generate efficiency, improve health and increase the satisfaction of professionals and patients. In this paper the historical process of implementing Management in the healthcare system the effects of evaluation in the health system are reviewed.
